Enrollment                                            

Our courses are offered to Doctors and/or Staff Members. 

Our definition of a Doctor is a dentist with a DDS or DMD licensed to practice dentistry in this country or any other country, whether as an individual, an associate, or an employee of a dental practice.  Lab Technicians will be enrolled as a Doctor.

Our definition of a Staff Member is an individual in a dental office acting as an office manager, dental assistant, registered dental assistant, or hygienist.  Doctors are not considered Staff Members.

Tuition

Payment in full is required to hold your place in the course.  All tuition is quoted and to be paid in US currency. 

While we do not anticipate any changes in 2008/2009 tuition; tuition increases may occur without notice.  Please confirm the current tuition when registering for a course.  Tuition for courses must be paid in full 30 days prior to the course date.

Payment

For your convenience we accept Visa, Master Card, Discover, Diner’s Club, American Express, Checks and Cash.  A $50 fee will be incurred for returned checks.

Course Availability

Dentistry By Design, Inc. reserves the right to change course dates up to 15 days before the scheduled start date and will refund all payments in such cases.

Course Cancellation Policy

Course Tuition Payment within 60 days of the scheduled date is final.  Cancellations between 60 and 90 days from course date are subject to a non-refundable $500 fee.  Cancellations over 90 days from the course date will receive a full refund.

Once enrollment is confirmed, Dentistry By Design, Inc does not honor any course cancellations.  Attendees who are unable to attend course will forfeit payments. No payments will be refunded or transferred to any other Dentistry By Design Course. Except as noted above, these policies are strictly enforced.

There are no post course refunds.

All sales are final.  California residents will be charged the applicable sales tax.  A $25 FedEx shipping and handling charge will be added to orders In the USA.  International shipping charges will vary upon location.  Receipts are responsible for payment of duties and taxes.  Special handling rates are available on request.
